Reason for Referral:

Intellectual disability, reliant on others for food choices, weight gain over last few months, management of chronic health conditions – diabetes, education for SIL staff re appropriate dietary intake and food / menu planning. 

Food Texture: 

Regular food cut up into 1.5 cm x 1.5 cm pieces 

Thin fluid

( As per speech pathologist- last reviewed in Sep 2019)

Medical History: 

- Complex genetic disorder (Chromosome 6 Deletion) -- risk of weight gain 

- Diabetes

- High Cholesterol

- Proteinuria

- Non aligning of upper and lower jaw

- Atrium Septum Defect

Nutrition Goals:

  • Ensure adequate nutrient intake and good blood glucose control 

  • Reduce weight to healthy weight range of 50 -57 kg; Height = 152 cm

    • Weight = 70.7 kg (on 17.11.2022) BMI = 30.6 kg/m2

    • Weight = 71.7 kg (on 19.01.2023), BMI = 31.0 kg/m2

  • Improve overall nutritional intake for weight loss

  • Improve food variety, eating for pleasure to improve the experience with eating 


Nutrition Recommendations:

  1. For better Glucose control and Weight Loss

    • Provide optimal carbohydrate portion at meals and snack for optimal blood glucose control

    • Provide at least ½ plate (or 2 cups) of salad to create a lower Glycaemic index meal and satiety

      • Raj likes raw onions and fresh raw garlic in salad dressing

      • If Raj is asking for second serve, encourage him to finish all his salad/vegetables first

    • Follow portion Plate Model

    • Provide a variety of meals and snack as per meal plan

    • Raj loves bread- only offer bread as per portion on meal plan (Meal time = max 2 slices; Snack time = 1 slice)

    • Raj likes most cuisines ( ie Italian, Thai, Indian, rice paper roll, sushi, curry etc), but does not like spicy foods

  2. Monitor weight fortnightly

  3. Commence Food Chart and Meal Photos

  4. Have adequate fluids throughout the day to help with bowel movements

    1. Encourage 2 L of water per day

    2. Continue Bowel Chart

  • Try not to offer food or snacks out side of above times.

  • Mindful of carbohydrate portion (For example, Rice = 1 cup cooked; Pasta = dried is 50g in weight; Bread = max 2 slices)

  • Aiming to allow 3-4 hours gap between any food intake

  • No dessert after dinner. Dessert only sometime (ie when eating at mum’s)

  • Always provide at least 2 cups of salad/vegetable at meal time. Encourage to finish salad/ vegetables before second serve. Mother reported Raj usually stops eating when he is full.

Pizza Wrap

  • Only use 1 egg and shredded chicken instead of 4 eggs

  • Use 1 low GP wrap/ Lebanese bread/ Roti as pizza base

  • Use total of 60g Vermicelli noodle (cooked) equally devided between 4 rolls

  • Add more thinly sliced lettuce, cucumber, carrots, bean sprouts, shallot to increase volumne

  • Put variety of toppings (salmon, pink ginger, nori, cucumber, grated carrots, shallot, avocado etc)
